Preston Sports Clinic Ltd (Company Number 13971995) trading from 35-36 Momentum Place, Bamber Bridge, Preston, Lancashire, PR5 6EF.
Within the context of this policy ‘Preston Sports Clinic’, ‘PSC’ and ‘We’ mean Preston Sports Clinic Ltd.
What information we collect?
Personal information provided by you
Electronic information - At the time of booking an appointment through our booking page, we collect personal information about you (your name, date of birth, email address and contact number).
Hand-written information (prior to March 2020) – We keep a paper record of each patient, containing personal details such as name, age, gender, contact information, GP name and surgery. We use this record to keep medical notes from each appointment at PSC, in accordance with our professional duties.
Personal information about other individuals
If you give us information on behalf of someone else, for example, booking an appointment on behalf of someone else, you confirm that the other person has appointed you to act on his/her behalf and has agreed that you can give consent on his/her behalf to the processing of his/her personal data.
Online Payments
Our online payments for bookings and purchases is powered by Stripe. By processing your payment through Stripe you are recognised by Stripe as a ‘End Customer’, data collected includes transaction data, identity and verification data. How will we use the information about you?
We collect information about you so that we can:
- Keep up to date accurate medical records on your medical history, assessments and treatments.
- We may use your contact details to contact you regarding any future appointments or to contact you regarding your progress with your recovery and ongoing home exercise plan.
- As we store health related records there is a minimum retention period. We store your records for a minimum of eight years from the date of your last appointment.
- Occasionally, we may send information about PSC and offers we may be running.
Who your information might be shared with
We use your personal data and medical records to provide the best possible care, other sports therapists or physiotherapists working under PSC may have access to your data when required. We do not share your personal or medical information with third parties.
It should be noted that we do use Mail Chimp as our email marketing software and we would refer you to their privacy policy and terms & conditions for further information in relation to their policies.
Keeping your data secure
Under GDPR we will ensure that your personal data is processed lawfully, fairly, and transparently, without adversely affecting your rights. We will only process your personal data if at least one of the following basis applies:
a) you have given consent to the processing of your personal data for one or more specific purposes;
b) processing is necessary for compliance with a legal obligation to which we are subject;
c) processing is necessary to protect the vital interests of you or of another natural person;
At Preston Sports Clinic we use Cliniko.com to process our bookings, personal and medical data. All data is stored securely in the the UK. Cliniko’s privacy policy can be viewed here. Cliniko’s security policy can be viewed here.
While we will use all reasonable efforts to safeguard your personal data, you acknowledge that the use of the internet is not entirely secure and for this reason we cannot guarantee the security and integrity of any personal data that are transferred from you or to you via the internet. What rights do you have?
Right to request a copy of your information
You can request a copy of your information which we hold (this is known as a subject access request). If you would like a copy of this, please make this request by email to hello@prestonsportsclinic.co.uk and let us know the information you want a copy of.
Right to correct any mistakes in your information
You can require us to correct any mistakes in your information which we hold free of charge. If you would like to do this, again please make this request by email to hello@prestonsportsclinic.co.uk and let us know the information that is incorrect and what it should be replaced with.
Right to erase of your data
You can ask us to remove all of your personal data from our systems. If you would like to do this, please email hello@prestonsportsclinic.co.uk and let us know you wish for your information to be removed. We do have a professional obligation to update and keep medical records on file.
Right to ask us to stop contacting you with direct marketing
You can ask us at any time to stop contacting you for direct marketing purposes. If you would like to do this, please either email hello@prestonsportsclinic.co.uk or click on the unsubscribe link at the bottom of our email correspondence.
